The best discovery experience is when leads from the whole cross-functional team are brought in before requirements are set. Stakeholders and Product Managers need to believe it is important, or be open to influence and reason. When requirements come just from customer executives, or worse, are self-referential to existing product thinking, it is easy for a product to veer off course. 

For learger organizations this graphic could use a step between F and G. Review with stakeholders is important, and at least receiving feedback after distribution of research findings is important.
